These Terms & Conditions ("Terms") govern your access to and use of the Ruh Revival website, booking systems, services, and related communications (collectively, the "Services"), operated by Ruh Revival LLC, a limited liability company organized under the laws of the Commonwealth of Pennsylvania.

By accessing or using our Services, you agree to be bound by these Terms. If you do not agree, please do not use our Services.

1. About Ruh Revival

Ruh Revival LLC ("Ruh Revival," "we," "our," or "us") provides wellness services including intravenous (IV) vitamin and hydration therapy, vitamin and nutrient injections, and red light therapy. All clinical services are delivered by licensed medical professionals under the supervision of a licensed physician serving as Medical Director.

Ruh Revival LLC is an independent entity. It is not affiliated with, owned by, or operated by any other medical practice, hospital, or healthcare organization that may operate at the same physical address. The presence of other healthcare providers at our location does not imply any affiliation, partnership, or shared clinical responsibility.

2. Eligibility

You must be at least 18 years of age to receive services. Individuals under 18 may receive services only with written parental or legal guardian consent and prior approval by the Medical Director of Ruh Revival.

3. Nature of Services

The wellness services offered by Ruh Revival are elective in nature. While our services are administered by licensed clinical professionals under physician oversight, they are not intended to serve as a substitute for evaluation, diagnosis, or treatment by your primary care physician or specialist.

A provider–patient relationship is established only after appropriate medical screening, informed consent, and in-person care delivery by Ruh Revival clinical staff.

Content on the Ruh Revival website and marketing materials is provided for informational and educational purposes only and does not constitute individualized medical advice. You should consult your personal physician regarding any medical conditions, medications, or health concerns before receiving services.

4. Appointments & Payments

• A valid payment method may be required to secure an appointment.
• Payment is due in full at the time of service unless otherwise specified in writing.
• Ruh Revival is a self-pay wellness practice. Services are generally not billed to health insurance.
• Prices, services, packages, and availability are subject to change without prior notice.
• Refunds, if applicable, are issued at the sole discretion of Ruh Revival and subject to the refund policy posted at the time of purchase.

5. Cancellation & No-Show Policy

To respect our clinical staff's time and ensure availability for all clients:

• Appointments must be canceled or rescheduled at least 24 hours in advance.
• Late cancellations (less than 24 hours' notice) may be charged a fee of up to 50% of the scheduled service.
• No-show appointments may be charged up to 100% of the service cost.
• Ruh Revival reserves the right to refuse future bookings for clients with repeated missed appointments.

6. Health Disclosure & Screening

All clients are required to complete a health intake and screening questionnaire prior to treatment. You represent and warrant that all health information you provide is truthful, accurate, and complete; you will disclose all current medications, supplements, known allergies, and medical conditions, including but not limited to pregnancy or nursing status, kidney disease, heart failure, electrolyte disorders, and history of anaphylaxis or allergic reactions; and you will promptly inform Ruh Revival staff of any changes to your health status before each visit.

Failure to disclose accurate and complete health information may result in adverse clinical outcomes. Ruh Revival LLC, its Medical Director, and its clinical staff shall not be held liable for complications arising from incomplete, inaccurate, or withheld health information.

Ruh Revival reserves the right to decline, modify, or discontinue any service based on clinical judgment, contraindications, or safety concerns. Your safety is our priority.

7. Informed Consent & Assumption of Risk

By choosing to receive services from Ruh Revival, you acknowledge and agree that participation in IV therapy, injections, and wellness services is entirely voluntary; you will be provided with, and are required to sign, a separate Informed Consent form prior to treatment that describes the specific risks, benefits, and alternatives for the services you will receive; and you understand the nature and purpose of the services being provided.

You acknowledge that IV therapy and injectable services carry inherent risks, which may include but are not limited to: pain, bruising, swelling, or bleeding at the injection or IV site; IV infiltration or extravasation (leakage of fluids into surrounding tissue); localized or systemic infection; phlebitis (inflammation of a vein); allergic reaction, including rare risk of anaphylaxis; nausea, dizziness, lightheadedness, or vasovagal response; air embolism (rare); and fluid overload or electrolyte imbalance, particularly in individuals with underlying cardiac or renal conditions.

By proceeding with treatment, you voluntarily assume all risks associated with the services, whether known or unknown, and agree that you have had the opportunity to ask questions and have received satisfactory answers.

8. Pregnancy & Nursing

Certain IV formulations and injectable therapies may be contraindicated during pregnancy or while nursing. You are required to disclose pregnancy or nursing status before receiving any services. Ruh Revival reserves the right to decline or modify services for pregnant or nursing clients based on clinical judgment and patient safety considerations.

9. Communication Consent

By providing your contact information to Ruh Revival, you consent to receive communications via phone, text message (SMS/MMS), and/or email for appointment confirmations, reminders, and follow-ups, and service updates, promotions, and wellness information.

You may opt out of marketing communications at any time by contacting us or using the unsubscribe mechanism provided. Transactional communications (appointment reminders, safety notices) may continue even after opting out of marketing messages. Message and data rates may apply. Ruh Revival is not responsible for charges from your mobile carrier.

10. Photo, Video & Testimonial Release

Ruh Revival may request your permission to photograph, video-record, or collect written or verbal testimonials for use in marketing, social media, educational, or promotional materials. Any such use requires a separate, signed Photo/Media Release form. You are never required to consent to photo, video, or testimonial use as a condition of receiving services.

11. Use of Website

You agree not to use the website for any unlawful purpose; attempt to interfere with the website's functionality, security, or infrastructure; or submit false, misleading, or fraudulent information.

12. Intellectual Property

All content on the Ruh Revival website, including text, images, logos, branding, and design, is the property of Ruh Revival LLC and may not be copied, reproduced, or distributed without prior written permission.

13. Limitation of Liability

To the fullest extent permitted by applicable law, Ruh Revival LLC, its owners, officers, Medical Director, employees, agents, and contractors shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from or related to your use of our Services, including but not limited to loss of profits, data, or goodwill.

Nothing in these Terms is intended to limit liability for gross negligence, willful misconduct, or any liability that cannot be excluded or limited under applicable law.

14. Indemnification

You agree to indemnify, defend, and hold harmless Ruh Revival LLC, its owners, officers, Medical Director, employees, and agents from and against any claims, damages, losses, liabilities, costs, or expenses (including reasonable attorneys' fees) arising from: (a) your breach of these Terms; (b) your provision of inaccurate or incomplete health information; or (c) your misuse of the Services.

15. Disclaimer of Warranties

All services and website content are provided "as is" and "as available" without warranties of any kind, express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, or non-infringement. Ruh Revival does not guarantee specific outcomes or results from any service.

16. Privacy & Health Information

Your use of our Services is also governed by our Privacy Policy. Ruh Revival LLC collects, uses, and safeguards personal and health information in accordance with applicable federal and state law, including HIPAA where applicable. Your health information will be used for treatment, payment, and healthcare operations, and will not be disclosed to third parties except as required or permitted by law. A separate Notice of Privacy Practices is provided to all clients and is available upon request.

17. Dispute Resolution (Arbitration Agreement)

Step 1: Informal Resolution. Before initiating any legal claim or proceeding, you agree to contact Ruh Revival at compliance@ruhrevival.com to attempt to resolve the dispute informally. Both parties agree to make a good-faith effort to resolve the matter within thirty (30) days.

Step 2: Binding Arbitration. If the dispute is not resolved informally within the period described above, it shall be resolved through binding arbitration rather than in court, conducted by a neutral arbitrator in accordance with the rules of the American Arbitration Association (AAA) or a similar recognized arbitration body, located in Lehigh County, Pennsylvania, and conducted in English. The arbitrator's decision will be final and binding.

Step 3: Class Action Waiver. You agree that any disputes will be resolved on an individual basis only. You waive any right to participate in a class action, collective action, or representative proceeding.

Step 4: Exceptions. Either party may seek injunctive or equitable relief in a court of competent jurisdiction for intellectual property violations or unauthorized access or misuse of systems.

18. Governing Law & Venue

These Terms shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania, without regard to conflict of law principles. Any legal action not subject to arbitration shall be brought exclusively in the state or federal courts located in Lehigh County, Pennsylvania.

19. Severability

If any provision of these Terms is held to be invalid, illegal, or unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect.

20. Entire Agreement

These Terms, together with the Privacy Policy, Informed Consent form, and any other documents expressly incorporated by reference, constitute the entire agreement between you and Ruh Revival LLC with respect to the Services.

21. Changes to Terms

Ruh Revival LLC reserves the right to update or modify these Terms at any time. Material changes will be communicated via our website or direct notification. Continued use of our Services following the posting of changes constitutes your acceptance of such changes.
